How Do I Enroll


Intake

To discuss whether Senior TLC is right for you or a family member, call us today at 704-874-0600. We will visit your home to assess your needs, answer your questions and explain our services. If you are interested in becoming a participant with us after this visit, the intake team will discuss your health care needs with other team members and arrange a visit for you at our PACE Day Center. Typically, one to two visits are necessary for you to get to know us and for us to get to know you and your special medical needs. We will ask your permission to get medical records from your other providers so we can have a comprehensive view of your specific situation.

 

Assessment

After each team member has reviewed your medical condition, we will meet to talk about the care and services you need. At this meeting, the individualized plan of care is developed.

 

Family Conference

After the team has discussed your health condition, a staff member will call you and your family, or someone close to you, to set up a meeting to discuss your best care options. If it is determined that you qualify and you are interested in participating in Senior TLC, you can enroll in our program. We will discuss at that time:

  • Your personal plan of care and the role your family/loved ones will play
  • Your monthly costs, if any

 Approval

The nursing facility pre-admission screening team will determine whether or not you meet the level of care criteria required to participate in Senior TLC. If you do not agree with the decision that is made by this screening team, you may appeal the decision.

Eligibility

Senior TLC serves residents of Gaston County and portions of Lincoln and Cleveland counties who are certified by the State of NC as eligible for nursing home care. You must be 55 or older and able to live safely in the community at the time of your enrollment.

 

Referral Guidelines

  • Are you or  your loved one 55 years of age or older?
  • Do you or your loved one need a skilled nursing home level of care?
  • Can you or your loved one live safely in the community with support services?
  • Do you or your loved one live in the following counties/zip codes?

Gaston County – all zip codes

Cleveland County – 28021, 28073, 28086, 28150

Lincoln County – 28006, 28021, 28033, 28080, 28164

A referral can also be made by a family member, physician or other provider who is familiar with your condition. To make a referral call 704-874-0600.
